New development: You can’t change Optional-Subject, exam-Centre,, Indian Language paper (Paper-A) and Answer Medium. You’ll have to stick to whatever you choices made in the prelims application form. (Ref: Instruction PDF file, page no. 3.)
- Last date is 11th November. MAKE NO HASTE in filling this form. Thoroughly read the PDF files uploaded on that page.
- Questions-queries post in comments. I request senior players to help others filling up the DAF-form.

Hi All,
I have one doubt in filling DAF.
I have applied only for Indian Forest Srvice exam in the last year Civil services(P) and i wrote the exam.
So while filling the number of times i have attended the Civil Service(P) exam now , should i enter it as ‘2’ or should it be only ‘1’ as i have only applied for “Forest service Exam” last year.

I think I have made a mistake in DAF. The column where it was asked to tell about the details of previous CSE(PRE) Attempts. I wrote about 2013s attempt but made a mistake and did not write about 2014s prelims. Now that I have submitted the form,please tell me if this can be a cause of rejection. I am very much scared. Please someone talk to me here.

You don’t need to get additional declation form. Its with in the daf in the form if a point given on the last page before summary sheet.

Guys, if you have made any mistake in filling the DAF and have submitted it, you need not worry. Just attach a letter to “Under Secretary, UPSC regaring the correction along with the DAF while submitting.
I have two questions
1) Do i have to send Photostat copies of only 10th and my degree or All the education qualification
2) Do I have to attach a residence proof??

1. Yes. Attested copies of both 10th and degree. I just self attested.
2. No need of residence proof
only attested copies of 10th and degree completion certificate… if you are from any category , then you have to send a photostat of those certificates. else nothing more…

Hello Sir…I have 03 queries: 01) instructions say that candidates are advised not to sign the summary sheet, which is a part of the DAF. however, there is a field of candidate’s signature in the same like the other pages of the DAF.please clarify. 02) as regards informing employer, a copy of intimation given to the employer should be sent or the signature is enough? though the instructions point towards signing on the declaration. 03) A humbug which i did is that i filled 1,2,3,4, etc in preference orders instead of 01,02,03,04…is this a issue?

guys i have a problem here.i tried registering but it shows that u have already registered and that a mail has been sent to my id which contains my login and id.i checked my mail(all folders including trash,spam). the last mail that i could find from noreply@upsc.nic.in is regarding prelims part 1 and 2 registration.can someone plz tell me wat is to be done?iv sent a complaint to the id mentioned on the site as well.

Hi Mrunal, I left the “Prizes” column blank and now when I read the instructions it says no column should be left blank. Will it create any problem later on?
I for one think that it should not be an issue. If it was mandatory they should have made entry into that column compulsory yet you don’t know for sure, Government organizations are usually very sensitive or I may say fussy about such issues.

I am facing problem in “Employment Information” section.
I have given my employment details.
At the end when I click Save and Continue, it is asking me to “Save and Add” in the employment section.
I did it. But it is again showing the same.
In the Final Submission it is showing Employment Information as not filled.
Is any one getting the same problem?

In the documents to be submitted along with DAF , for education qualification certificates- If one is having masters degree too then is there any need to submit it or only the qualifying graduation degree copy has to be submitted? Also I believe- only 10th 12th Grad degree (and NOT marksheets!!) is to be sent along with DAF copy.
Please correct me if I am wrong!!
